Admissions Procedure
The
following procedure is followed for every prospective Resident:
-
Background and personal information is sought
about perspective residents. To assist the collation of basic information,
a personal record form is completed. This has the advantage also of
forming the basic record of a resident throughout any placement.

A Brochure giving information about the Home is
provided to all interested parties.
-
Where possible an indication of the Maintenance
Charge is provided. This may vary according to the particular needs of
the prospective resident.
-
All persons involved in previous and current care are contacted personally
to form some preliminary assessment of the particular needs of the
prospective resident.
-
Assuming suitability is accepted by all parties,
visits to the Home are encouraged and welcome. These visits should occur
prior to a decision on suitability being agreed. Over night stays are
also encouraged after a day visit.

The Care Manager is
expected to have identified in written terms the needs of the prospective
Resident and during the course of enquiries, to be reasonably satisfied
that the Home can meet those needs.

The
staff, together with the Care Manager, Parent, Guardian and prospective
Resident, will agree on an initial Care Plan which will consist of
requirements of basic care, Occupational Therapy activities, guidelines on
requirements to cope with disturbed behaviour or medical emergencies and
identify preliminary objectives.

A trial period usually of six months is agreed.
This is capable of being varied according to circumstances.

A contract is entered into with the appropriate
parties regarding Maintenance charges, frequency of payment and revision
arrangements.

Medical
requirements, for example the Home will require a ‘Free From Infection’
Certificate signed by a GP stating that the prospective Resident has not
been exposed to any infectious diseases in the last two weeks.

The Home will also require evidence of a ‘clear’ stool specimen that is
negative for intestinal pathogens, cysts and ova.

Where the Resident has for 6 months or more, been in a long stay
establishment, results of a blood test showing the Hep.B. status is
necessary. These results should be dated not more than seven days prior
to admission and must be satisfied and confirmed in writing before the
prospective resident will be accepted into the Home. These are simple
but necessary precautions to seek to avoid the spread of any infection.

During the trial period a more detailed assessment of the needs of the
Resident will be produced in conjunction with all parties including the
Resident

Before the end of the trial period, a comprehensive review will take place
again involving all parties when a decision will be taken to terminate,
extend trial period or confirm as a permanent placement.
-
All permanent placements are reviewed at six monthly intervals or more,
frequently if necessary, again with all parties being involved. A written
report is produced and circulated to all concerned.
